Request for Letter of Non-Contagious Disease

Embassies or consulates may require a Letter of Non-Contagious Disease before allowing shipment of a deceased body to another country to ensure human remains imported do not contain an infectious disease that could threaten public health. Funeral homes may request a Letter of Non-Contagious Disease from the Montgomery County Public Health Department by emailing the required supporting documents to the Epidemiology and Preparedness Division Manager.

Once all required documentation is received, please allow at least 3 business days of processing time for each letter request. The completed letter will be emailed to the email address provided by the requester. If a hard copy of the letter or the presence of a notary is required, a meeting with the Epidemiology and Preparedness Division Manager must be scheduled.

 

 

Instructions

When requesting a letter, you must include the following information and supporting documents in order for your request to be processed.

  • Requester Name
  • Requester Email Address
  • Requester Phone Number
  • Indicate whether hard copy or notary is required for the letter. Please note, if this information is not provided, the completed letter will be emailed to the email address provided by the requester.
  • Copy of Death Certificate of Deceased
  • Embalming Record or Certificate of Cremation of Deceased
  • Funeral Home Name, Address, and Phone Number
  • Destination Country
  • Date Human Remains Will be Transported to Destination Country
  • To Whom the Letter Should Be Addressed To (e.g. Consulate General of ____)
    • Include the Address of the Consulate or Embassy
